(700 WLW)- The Ohio State Highway Patrol is currently investigating a two vehicle fatal crash that occurred on SR 133 near US 50 in Jackson Township, Clermont County at approximately 2:00 p.m. Thursday afternoon.
A 2011 Ford F-250, operated by Jason A. Lewis, age 34, of Grayson, KY was traveling northbound on SR 133. His vehicle was also occupied by Daniel Hensley, age 38, of Grayson, KY, Taylor Combs, age 20, of Olive Hill, KY, and Shelton Kouns, age 19, of Olive Hill, KY. A 1996 Ford F-350, operated by Chad W. Burns, age 38, of Peebles, OH was traveling southbound on SR 133. His vehicle was also occupied by Jeremy Burns, age, 33, of Peebles, OH. Preliminary investigation shows that Mr. Lewis failed to yield while turning left into a private drive and was struck by Mr. Burns. Mr. Burn’s vehicle traveled off the right side of the roadway and overturned.
Chad W. Burns was pronounced deceased at the scene. Jeremy Burns was transported to Bethesda North Hospital by Wayne Township EMS with minor injuries. Jason A. Lewis was not injured. Daniel Hensley, Taylor Combs, and Shelton Kouns were all treated on scene by Wayne Township EMS for minor injuries. The crash remains under investigation by the Ohio State Highway Patrol – Batavia Post.
